All requests for patient medical records require a Release of Medical Records form completed, signed, and must include the patient or guardian’s phone number, mailing address, and email address. Once the full retrieval fees and archive fees are determined by Access Corp, then instructions for payment of your scanned medical record will be sent via email. All prior requests for records must have been turned in by May 30th, 2019. All inquiries past this date for medical records will be submitted to medical archive company Access Corporation. Requests submitted will be addressed every two weeks, and may take 6-8 weeks to obtain. Once the medical record for a patient has been retrieved, the chart(s) will be scanned to produce an electronic copy. Instructions will be sent on how to obtain that electronic copy, and will be released after receipt of payment for the chart storage, retrieval, scanning, and secure destruction of the paper chart after 7 years.
