Medical Record Retrieval

Dear Patients and Families:

This letter is sent to inform you and/or your family that after almost 25 years of patient care here in Scottsdale, I will be closing my private practice and moving out of state.  I wish to extend my gratefulness to all of you and your families how much we have enjoyed getting to know you and help improve your health over the years.

We anticipate that our last date of appointments will be on or about the date of Friday, June 14th, 2019.  All requests for medication refills to cover up to 90 days’ supply must be submitted via email or phone before June 1st, 2019 (PLEASE specify local pharmacy 30 day fills versus 90 mail order).  Requests for copies of your medical records to be faxed or copied will need to be submitted no later than May 15th, 2019 (standard copy fees and mailing fees will apply, depending upon the size of your chart).  To minimize cost and time to send records, we recommend our sending your last three progress notes and lab work (as opposed to your whole chart).  We can also forward/fax copies of your records to your new physician with the included Release of Records form that you can print off of my website under the “forms” tab:  www.EricGreenmanMD.com  Copies of your medical records will be stored with secure storage company:  Access Corp., 3919 West Washington Street, Phoenix, AZ 85009, USA.  All requests after June 1st will need to follow directions on Dr. Greenman’s website and may take 4-6 weeks to process (additional access and copy fees will apply).
